Health Office

The Health Office is located across from the main office.  If a student is ill or injured, the student must check in at the Health Office to be assessed.  If the student needs to go home, or needs further follow-up care, the parent/guardian or emergency contact will be called to pick up the student.  The student must be checked out at the front desk, and I.D. verification is required.


Every student must have a completed Emergency Health Information Card with current contact numbers that are kept on file in the Health Office.  Also, on this card is an area to inform us of any special needs or medical conditions that pertain to your child.  Students requiring prescription medications to be given by the Health Office staff during the school day will need the physician to complete an “APS Provider Order and Medication Authorization Form.”  If a parent/guardian wants a student to carry a dose of over-the-counter medication, a “Parent Authorization Form” is available in the Health Office for the parent to complete, and to be kept on file in the Health Office.


Students must have all required immunizations, or a completed Certificate of Exemption, upon registering for school.  The student’s schedule will be held until the immunization requirement is completed.  Please call the Health Office if you have questions:  299-4735 ext. 30839
